This proverb highlights the extreme hardships faced by the poor, suggesting that their only source of comfort or luxury (symbolized by drinking butter) is found in dreams. It emphasizes that reality is so harsh for them that if they couldn't even find solace in their dreams, their suffering ('itchiness') would be unbearable or destroy them. It underscores the severity of poverty and the importance of even imagined comfort.
