This proverb means that something a person has kept or gotten accustomed to for a long time, whether good or bad, will become their distinguishing mark or characteristic. Even though a goiter is an illness, if a person has lived with it for a long time, it becomes a part of their appearance that differentiates them from others. This suggests that the choices, habits, or burdens a person carries throughout their life shape their identity, and others will come to know them by these traits.
